Is DSHS form 10-638 still required now that Meaningful Day has ended?
Not for Meaningful Day. DSHS eliminated the adult family home Meaningful Day add-on effective July 1, 2025, and form 10-638, the monthly activities and challenging behavior log, now carries a notice that Meaningful Day is no longer available. Your other documentation duties, including the negotiated care plan and the incident log, still apply.
What changed
In a June 6, 2025 letter to adult family home providers, DSHS said the Legislature eliminated the Meaningful Day add-on in the 2025–2027 operating budget (DSHS letter AFH #2025-024). Form 10-638 (revised August 2025) now opens with a notice that, effective July 1, 2025, Meaningful Day is no longer available (DSHS 10-638).
What the form tracked
Form 10-638 was a monthly log. Week by week, it recorded:
- how many in-home and community activities the resident attended, and whether they participated and enjoyed them
- two targeted challenging behaviors: whether each happened, and whether it was disruptive
- which activities worked well that week
What still applies
Ending the add-on didn't change the core rules:
- Where needed, the negotiated care plan includes steps to reduce tension, agitation and problem behaviors (WAC 388-76-10355), and the home must carry it out (WAC 388-76-10365).
- The assessment covers behaviors, mood and refusal of care (WAC 388-76-10335), and it's updated after a significant change.
- The incident log and reporting duties are unchanged (WAC 388-76-10220).
If a resident receives other DSHS-contracted behavior services, that contract sets its own documentation requirements. Check it and ask the case manager.
Is the idea still worth keeping?
Tracking which activities a resident enjoys, and how often specific behaviors happen, still helps at care plan reviews, even though the form is no longer required. Daily notes that say what happened, what staff did and how the resident responded give you that picture without a separate monthly form.
